Tameside Council is looking for a dedicated Adult s Social Worker to join their Integrated Urgent Care Team. This is an exciting opportunity to be part of a team at the forefront of transformation and investment in adult services and social work practice.

As an Adult s Social Worker, you will take the lead in supporting adults to live well at home, with a focus on preventing hospital admissions and supporting carers to avoid breakdowns. Your work will be impactful, and you will have the opportunity to contribute to the ongoing development of services in this dynamic team.

• Manage a caseload of adults requiring urgent assessments or support.
• Conduct Care Act Assessments, Support Plans, Mental Capacity Assessments, and Best Interest Decisions.
• Signpost individuals to both internal and external services to enhance wellbeing and independence.
• Provide critical support to adults and carers to maintain care at home and prevent hospital admissions.

• Proven experience in adult social work, particularly in urgent care settings.
• Ability to manage complex cases and deliver person-centred care.
• SWE Registration is required.
