Salary: £35,392 – £45,996 (pro rata depending on experience)

Hours: 30 part time (37.5 full time)

QEF Care and Rehab Centre (CRC) is a state of the art 48 bedded rehab centre. We offer four different client pathways to include Neuro Rehabilitation, General Rehabilitation, Complex Neurological Care and Out Patient / Respite Services

Job purpose

  • The main purpose of the role is to work as an autonomous practitioner within the QEF Care and Rehabilitation Centre working closely with acute services, GPs and those who refer clients for consideration to our inpatient rehabilitation services.
  • You will be responsible for fulfilling the service clinical triage requirements and will need to undertake assessments of clients with a range of clinical presentations, many of which will have a neurological diagnosis or condition.
  • You will need to use specialist clinical reasoning, evidence based practice, enhanced assessment, problem solving and decision-making skills, to identify the client needs, determine suitability for our service and make recommendations on the clinical pathway the client will most benefit from.
  • This will be a crucial role in the development of a trusted assessor process with those referring into our Care and Rehab Service

Role Responsibilities

  • To be professionally and legally accountable for all aspects for your own and delegated work.
  • To work within codes of practice and professional guidelines.
  • To undertake specialist holistic rehab assessment (including those with complex presentations and multi pathologies) making use of clinical reasoning skills. This may include face assessment or telephone assessment / triage.
  • To control and manage triage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
  • To evaluate patient progress, write reports and assessments and being responsible for the handover
  • To effectively communicate with patients, family members, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication, e.g. hearing loss, diminished sight, depression, aphasia, speech problems, cognitive impairment, behavioural problems and pain
  • To work in line with QEF policies and procedures.
